{"id":973,"date":"2021-06-16T01:06:56","date_gmt":"2021-06-16T01:06:56","guid":{"rendered":"https:\/\/msapc.managed3.brouser.net\/hand-center\/?page_id=973"},"modified":"2021-12-30T16:09:54","modified_gmt":"2021-12-30T16:09:54","slug":"boutonniere-deformity","status":"publish","type":"page","link":"https:\/\/msapc.com\/hand-center\/hand-conditions\/boutonniere-deformity\/","title":{"rendered":"Boutonniere Deformity"},"content":{"rendered":"<h3>Definition<\/h3>\n<p>Disruption of the central slip\/band that creates a flexion posture of the proximal interphalangeal joint (middle joint) with concurrent hyperextension posture of the distal interphalangeal joint (furthest joint) due to pathology at the central slip and triangular ligaments.<\/p>\n<h3>Symptoms<\/h3>\n<ul>\n<li>Pain and deformity of the finger<\/li>\n<li>Inability to flex the DIP (tip) of finger and extend (straighten) the PIP (middle joint).<\/li>\n<\/ul>\n<h3>Pathology<\/h3>\n<ul>\n<li>Disruption of the central slip and triangular ligaments.<\/li>\n<li>Traumatic (laceration\/cut, direct blow to tip of finger).<\/li>\n<li>Degenerative (chronic inflammation, Rheumatoid arthritis)<\/li>\n<li>Unopposed lumbricals and collateral bands pull the middle joint (PIP) into flexion and the last joint (DIP) into extension.<\/li>\n<li>Pathology creates deformity of the finger and lack of or inability to extend middle joint<\/li>\n<\/ul>\n<h3>Treatment<\/h3>\n<p><strong>Non-surgical<\/strong><\/p>\n<ul>\n<li>Early management with strict splinting of the PIP (middle) joint for 3-6 weeks.<\/li>\n<li>Occupation Therapy<\/li>\n<\/ul>\n<p><strong>Surgical<\/strong><\/p>\n<ul>\n<li>Indicated when non-operative treatment fails.<\/li>\n<li>Open tendon injuries and\/or PIP (middle) joint injuries that result in an extension deficit of 30 degrees or greater.<\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>Definition Disruption of the central slip\/band that creates a flexion posture of the proximal interphalangeal joint (middle joint) with concurrent hyperextension posture of the distal interphalangeal joint (furthest joint) due to pathology at the central slip and triangular ligaments.
