{"id":77,"date":"2021-05-28T19:04:26","date_gmt":"2021-05-28T19:04:26","guid":{"rendered":"https:\/\/msapc.managed3.brouser.net\/hand-center\/?page_id=77"},"modified":"2021-12-30T16:08:21","modified_gmt":"2021-12-30T16:08:21","slug":"scapho-lunate-advanced-collapse-slac","status":"publish","type":"page","link":"https:\/\/msapc.com\/hand-center\/hand-conditions\/scapho-lunate-advanced-collapse-slac\/","title":{"rendered":"Scapho-lunate Advanced Collapse (SLAC)"},"content":{"rendered":"<h3>Definition<\/h3>\n<p>Advanced arthritis of the radiocarpal and midcarpal joints due to progressive instability\/dissociation between the scaphoid and lunate (carpal) bones.<\/p>\n<h3>Symptoms<\/h3>\n<ul>\n<li>Dorsal (top side) wrist joint pain localized over the region of the scapholunate interval.<\/li>\n<li>Progressive wrist stiffness and weakness.<\/li>\n<li>Increased pain with weight bearing across the wrist (push-up activity).<\/li>\n<\/ul>\n<h3>Pathology<\/h3>\n<ul>\n<li>Chronic ligament instability between the scaphoid and lunate creates a DISI (dorsal intercalated segment instability) deformity.\n<ul>\n<li>Scaphoid flexed and lunate extended<\/li>\n<li>Can progress to capitolunate joint (capitate and lunate carpal bones).<\/li>\n<\/ul>\n<\/li>\n<li>X-rays\n<ul>\n<li>PA views demonstrate scapholunate widening (separation &gt; 3mm), joint space narrowing between the scaphoid and radial styloid and sclerosis (increased bone density\/hardening).<\/li>\n<li>Lateral views demonstrate DISI deformity (flexed scaphoid and extended lunate).<\/li>\n<\/ul>\n<\/li>\n<li>Radiolunate joint is spared<\/li>\n<\/ul>\n<h3>Treatment<\/h3>\n<p><strong>Non-surgical<\/strong><\/p>\n<ul>\n<li>Rest, activity modifications, bracing, ice and NSAIDs (ibuprofen)<\/li>\n<li>Corticosteroid injection<\/li>\n<\/ul>\n<p><strong>Surgical<\/strong><\/p>\n<ul>\n<li>Reconstructive surgery involves excising the scaphoid and arthrodesis and is indicated when non-operative treatment fails.<\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>Definition Advanced arthritis of the radiocarpal and midcarpal joints due to progressive instability\/dissociation between the scaphoid and lunate (carpal) bones.
